PLEASE NOTE: Successful completion of all four stages of required field experience requires communication skills sufficient for entry level in to the profession. If communication skills are not sufficient, as determined by the Speech, Language, and Hearing Screening, remediation through the Speech and Hearing Clinic may be required.

Teacher Candidate Profile - All teacher candidates who will be student teaching should complete this form and bring it to the Student Teacher Orientation for their University Supervisor.

Confidentiality Form for Rehabilitative Sciences Students -
All rehabilitative Sciences students must complete this form and submit it to the OFS, 102 Stevens Hall, before beginning the internship.

Letter of Introduction -
All teacher candidates, who are entering block, must complete this form and return it to the OFS, 102 Stevens Hall, before beginning Stage 3 Field Experiences.

Information on Medical Clearances and TB screening is found on the Clearance link. The TB is for two years. The physical is valid for one year. 

  • Liability Insurance:
    This insurance is required prior to the beginning of the actual classroom field experience. This coverage can be obtained through personal liability insurance or by purchasing insurance through one of the links below. The liability requirement coverage is $1,000,000. Suggested sites:

  • Health Insurance:
    This insurance is preferred but not required prior to the beginning of the actual classroom field experience. If the district or agency requires this coverage, the teacher candidate will need to have it. Most teacher candidates already have health coverage during their college experience. If a teacher candidate needs this coverage, information can be obtained through the OFS or at the Becht Health Center. The University endorses the Consolidated Health Plan.   • Disposition Policy and STPP Rubric
    This is used by the University Supervisors and the Cooperating teachers to determine the professional disposition of the teacher candidate.
